This questionnaire and any attachments are strictly confidential and will be used solely for appraisal by Parks Maintenance Inc.  However, it is not to be construed as granting open account terms, as this decision is reserved by the credit department of Parks Maintenance Inc.  Permission is hereby granted by the undersigned for Parks Maintenance Inc., to contact any form or individual regarding information deemed necessary to arrive at a credit decision.

Customer to pay for a service charge of 1.5% per month (18% per annum) on all past due balances, in the events of any third parties are involved to collect any outstanding monies due by said business, the undersigned agrees to pay any reasonable collection agency cost, including attorney fees, whether or not litigation has commenced, and all costs of litigation incurred. The undersigned represents that he/she has the authority to execute this credit agreement on behalf the said business.
